Overview of Singapore's Community Care Sector

Community Care services are typically required for persons who need further care after being discharged from an acute hospital, as well as community-dwelling seniors who may be frail and need someone to watch over them or help them with their daily needs, to enable them to age well in the neighbourhoods they are familiar with. Community Care services are provided in community hospitals, home care, centre-based care facilities and long-term residential care facilities.

AWWA is a social service organisation that serves persons of different demographics across life- stages. Services include early intervention for pre-schoolers, education and disability support for children and adults with additional needs, assistance to low income families, caregivers, and health and social assistance for vulnerable seniors. AWWA is Singapore-based institution of a Public Character (IPC) status.

Established in 1987, the ECON healthcare group celebrates more than 30 years of commitment to quality healthcare and eldercare services. As a regional provider for Integrated healthcare services in Singapore, Malaysia and China, the group incorporates a chain of Medicare centres and nursing homes, hospital services, a care skill training centre, a chain of senior services centres, home care services, ambulance services, physiotherapy services and traditional Chinese medicine services. The group also provides consultancy for the development and management of retirement living communities.

Kwong Wai Shui Hospital boasts over 600 beds in a 12 storey nursing home building. Staffed by 400 trained and professional employees, our in-patient department (IPD), rehabilitation center and traditional Chinese medicine (TCM) center serves thousands of patients every month.

Despite being a charitable organisation that is heavily dependent on public support and donations, they remain focused on providing quality care and services to their clients.

Established in the mid-1960s, Lee Ah Mooi Old Age Home is a nursing home that cares for those who are unable to care for themselves or are unable to be cared for in their own homes.

They serve over 110 residents at the Thomson branch and over 130 residents at the Silat Branch.

NTUC Health Co-operative Limited (NTUC Health) is an NTUC social enterprise that provides comprehensive and integrated suite of quality and affordable health and eldercare services to meet the growing needs of families and their dependents. Building on more than four decades if experience and expertise, NTUC Health is among the largest senior day care, nursing home and home personal care providers in Singapore. It also offers other servicers for seniors such as an active ageing hub senior activity centres, community support for vulnerable seniors, and a sheltered/senior group home. In addition, it runs a chain of dental clinics and family medicine clinic.

St. Andrew's Mission Hospital (SAMH) first began as a free dispensary for poor local and migrant women and children in 1913.

Today SAMH operates St. Andrew's Community Hospital, St. Andrew's Autism Centre, St. Andrew's Mission Hospital clinics, St. Andrew's Nursing Homes at Buangkok, Henderson and Queenstown and St. Andrew's Senior Care centres at Kampong Glam, Tampines, Henderson and Queenstown. St. John's-St. Margaret's Nursing Home and Senior Care Center at Dover Avenue will begin operations in the first quarter of 2021.

St Luke's ElderCare is committed to caring for seniors in the community through services provide at 23 senior care centers and homes of the elders. They deliver a full range of integrated services and programmes for the elders that include center-based, community-based and residential-based services.

These services are designed to benefit elders who have suffered physical disabilities as a result of stroke, fall or accident, as well as the frail and lonely.

St Luke's Hospital, named after the patron saint of the medical profession, was the first hospital in Singapore dedicated to the elderly sick.

We have expanded our services beyond the elderly to enrich more lives. An institution of a Public Character, we care for 2,000 inpatients and 3,000 outpatients each year, regardless of race, language or religion. As illness may be long and chronic, we care holistically for patients' physical, emotional and psychosocial well-being.

Impactful and innovative in transforming community care, we were the first recipient of the President's Award for Social impact (2012), and the first community hospital to receive the National Healthcare Innovation and Productivity Medal (National University Hospital –St Luke's Hospital Integrated Path, 2016). We topped the Community Hospital Patient Experience Survey in 2018.

Being the forerunner in providing community dialysis treatment. The National Kidney Foundation (NKF) has embarked on an integrated renal care model to support the evolving health and social needs of our patient. This is especially crucial as patients are getting older and frailer, and their needs continue to become more complex, compounded by multiple chronic conditions such as cardiovascular disease, diabetes, hypertension and obesity.

In our roles as a community care integrator, we aim to right-site all our patients at a dialysis centre within 5km of their residence as part of our community integration efforts to actively engage the grassroots and volunteers to have greater interaction with our patients. We continue to adopt a collaborative approach by working closely with government agencies, social agencies and restructured hospitals to provide seamless, integrated renal care; levering on the strength of our volunteers; partnering individuals and corporates who give generously to our cause. We are continuing with our strategic move to set up dialysis facilities in community hospitals as we provide integrated kidney care in the community.

Vanguard Healthcare is a healthcare organisation set up by the Ministry of Health under MOH Holdings in 2015. Pearl's Hill Care Home, Woodlands Care Home, Tampines Care Home and Bukit Batok Care Home are managed by Vanguard Healthcare as well.

Vanguard Healthcare aims to contribute to the growth and development of eldercare services in Singapore through introducing enhanced care models for nursing homes and promulgating good practices, and innovations to other so as to benefit the overall nursing home sector over time.
